How to get your GP record

Your GP record includes information like any conditions or allergies you have and any medicine you're taking.

Most patients will automatically be given access to more information added to their GP record from November 2022 onwards. This includes letters, test results and appointment notes.

Some people may also have access to information added to their GP record before this date.

You can access your GP record and nominate someone you trust to access it too.

Access Your NHS Account

You must be aged 13 or over to access your NHS account. Find out more about who can use the NHS App.

You can have access via the NHS App or log in through the NHS website. 

NHS App

Download the NHS App on your smartphone or tablet.

If you're aged 13 to 15 years old then you need to complete our Online Access Application first & await approval.

You can also access the same services in a web browser by logging in through the NHS website.

By speaking to your GP surgery

you can ask for your GP record at your GP surgery via Subject access form

Proxy Access

You can apply for Proxy Access to another patient's record if you are their carer or they cannot manage online access themselves.

Access to children's records is usually only provided up to the age of 11 unless special circumstances apply.

Digital Skills Support

If you need help with technology including the internet, or you know someone who does, there is free help available at Well Newham.

They can help provide support for Online admin, Help filling out forms,  & Public computers and Wi-Fi access.
